Healing Streams Care Services, LLC is licensed to deliver Home and Community-Based Services in Minnesota for adults and people with disabilities. Our professionals assist individuals eligible for Minnesota's Waiver Programs so they can be catered to under any of the services we provide based on their needs.
We value working closely with individuals and families every step of the way from assessing their needs to providing the necessary supports and meeting agreed-on outcomes so they can live happily, comfortably and independently in their community.

Comprehensive care services tailored to individual needs and preferences
Individualized Home Supports are services for people who live in their own home or family home.
We provide short-term in-home respite care to individuals when their main care giver is away or needs a break.
Homemaker services help a person manage general cleaning and household activities.
We offer assistance and support to older adults who need reminders or physical assistance to remain in their own homes.
Companion Services comprise non-medical care, supervision and socialization for adults 18 years or older.
We help people with disabilities and seniors find and keep housing.
